[HCDX] SIO - the definitive statement?

Being old-fashioned, I like to use SINPO.

Recently SIO has been rearing its head. This is okay except for one thing -

SIO - does it officially (or as official as we listeners ever get) use 
values of
1-5 for each category or only 2-4 for each category?

I have seen documents that state both and I'd like to resolve this so I know
what the correct usage of this is.
